Introduction

This article details the data migration request form that FMX uses to gather the information essential to beginning the migration process. Ensuring that this data is correct is essential for making sure that this process runs smoothly, so please use this article to answer any questions you may have while filling out this form!

Please select the request type that most resembles your current situation. The name of the request will be automatically created based on this selection, so need to name your request!

*What is your email Address? 

This is important for us to be able to reach you if we have any issues during the data gathering process. Please make sure to include a valid email address that we can reach you at! 

Please note: You will have the opportunity to add other email addresses at the end of the request, so only include your email in this space!

Please include the name of the organization that is having the migration completed. 

FMX Access Requirements

Check this box if your organization use Active Directory for logging into your current system.

If you use Active Directory, please work with your IT Department to grant FMX access to your current software. If you have questions about domain or need a specific email address, please reach out to your Customer Success Manager to get you those details! 

Check this box if your organization uses a different Single Sign On to login to your current system.

Using your Customer Data Specialists email address to create an account

If you plan on using your Customer Data Specialists email address to create a new account, please see the following:

  • You will need to ensure that you include a unique identifier in the email address to ensure that they have access to your current software. The simplest way to do this will be to add "+random number" to the end of the email address. It should look like this:
    • first.last+######@gofmx.com
  • Below is a randomly generated number, please use this as the unique identifier within your Customer Data Specialists email address. Please make sure the include this as the email when filling out your data migration request.

When will you lose access to the software you are migrating from?

Please include the final date that you have access to this software, commonly this is the last day of the month. 

Please note: FMX requires a minimum notice of 10 business days (M-F) prior to the date you lose access to the 3rd party software to allow our Customer Data Specialist ample time to verify access to the system and extract all your data. If you do not grant us access in a timely manner, you may be held responsible for extracting the data.

Desired Active Pull Date

This is the date that FMX will enter the software to export all active request data. This will be the final time that FMX will enter the software, so make sure that all information is up to date prior to the date set. 

Please note this date must be within 60 days of your kick-off with FMX. 

The standard timeline for the completed migration of active data is up to 30 business days from this date, so picking a date prior to the date you lose access is highly encouraged, if at all possible.

Should Requests be imported as 'Pending Approval' or 'Already Approved'?

If you select 'Pending Approval' then all requests will need to be manually approved. 

Additional Details

Please include names and emails for all who will be involved in the data migration process within your organization.

This is important to ensure that the data is properly reviewed, and any formatting questions can be answered in a timely manner.

Below is a timeline of the Data Migration process, please review this to understand what can be expected during this period. 

Before the Data Review Meeting

  1. Data Migration Request form is submitted
  2. FMX Gather Data from software being migrated from

During Data Review Meeting

  1. Data is reviewed together and questions are answered
  2. Timeline is set and agreed upon based on implementation timeline and organization needs
  3. Any questions are answered

After the Data Review Meeting

  1. Static Data is formatted and confirmed by organization
  2. Static data is imported to FMX within 15 business days of Data Review Meeting
  3. Active Data is pulled on agreed upon date, formatted, and reviewed by organization
  4. Active Data is imported to FMX within 30 business days of active pull date
